{"id":964,"date":"2024-05-23T11:49:14","date_gmt":"2024-05-23T09:49:14","guid":{"rendered":"https:\/\/www.uni.lu\/lcsb-fr\/?post_type=news&#038;p=964"},"modified":"2024-11-06T10:27:16","modified_gmt":"2024-11-06T09:27:16","slug":"scores-de-risque-genetiques-et-medecine-de-precision-pour-parkinson","status":"publish","type":"news","link":"https:\/\/www.uni.lu\/lcsb-fr\/news\/scores-de-risque-genetiques-et-medecine-de-precision-pour-parkinson\/","title":{"rendered":"Scores de risque g\u00e9n\u00e9tiques et m\u00e9decine de pr\u00e9cision pour Parkinson"},"content":{"rendered":"\n<section class=\"wp-block-unilux-blocks-free-section section\"><div class=\"container xl:max-w-screen-xl\">\n<p>Une \u00e9quipe interdisciplinaire du <a href=\"https:\/\/www.uni.lu\/lcsb-en\/\" target=\"_blank\" rel=\"noreferrer noopener\">Luxembourg Centre for Systems Biomedicine<\/a> (LCSB) de l&rsquo;Universit\u00e9 du Luxembourg a \u00e9tabli qu&rsquo;un ensemble de petites variations sur diff\u00e9rents g\u00e8nes r\u00e9gulant les mitochondries, un composant important des cellules humaines, est associ\u00e9 \u00e0 un risque plus \u00e9lev\u00e9 pour la maladie de Parkinson. Pour la premi\u00e8re fois, les chercheurs ont aussi observ\u00e9 une correspondance entre ces pr\u00e9dictions g\u00e9n\u00e9tiques et les caract\u00e9ristiques de mod\u00e8les cellulaires : les cellules issues de patients ayant un score de risque polyg\u00e9nique \u00e9lev\u00e9 pr\u00e9sentant un dysfonctionnement mitochondrial. En utilisant les donn\u00e9es d&rsquo;un essai clinique r\u00e9alis\u00e9 par des collaborateurs internationaux, ils ont \u00e9galement montr\u00e9 que les patients avec un score de risque \u00e9lev\u00e9 r\u00e9agissaient mieux \u00e0 un traitement ciblant les mitochondries. Ces r\u00e9sultats, <a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/10.1002\/ana.26949\" target=\"_blank\" rel=\"noreferrer noopener\">r\u00e9cemment publi\u00e9s dans <em>Annals of Neurology<\/em><\/a>, ouvrent la voie pour une meilleure conception des essais cliniques et auront un impact consid\u00e9rable sur la m\u00e9decine de pr\u00e9cision pour la maladie de Parkinson.<\/p>\n\n\n\n<p>La maladie de Parkinson est le trouble neurod\u00e9g\u00e9n\u00e9ratif qui conna\u00eet la croissance la plus rapide, touchant environ 2 % de la population \u00e2g\u00e9e de plus de 60 ans. Comprendre l&rsquo;architecture g\u00e9n\u00e9tique complexe de cette maladie reste encore un d\u00e9fi. Les formes familiales dues \u00e0 une seule mutation repr\u00e9sentent 5 \u00e0 10 % des cas mais des facteurs g\u00e9n\u00e9tiques contribuent sans doute chez beaucoup d\u2019autres patients. Des \u00e9tudes sugg\u00e8rent en effet qu&rsquo;une combinaison de petites variations, appel\u00e9es variants, sur plusieurs g\u00e8nes pourrait constituer un facteur de risque pour la maladie. C\u2019est pour explorer l\u2019impact conjoint de ces variants, que la communaut\u00e9 scientifique a d\u00e9velopp\u00e9 ces derni\u00e8res ann\u00e9es des approches bas\u00e9es sur des scores de risque polyg\u00e9niques.<\/p>\n\n\n\n<h4 class=\"has-text-align-left wp-block-unilux-blocks-heading\"        id=\"le-risque-polygenique-mesurer-leffet-conjoint-de-plusieurs-variants-genetiques\"\n    >\nLe risque polyg\u00e9nique : mesurer l&rsquo;effet conjoint de plusieurs variants g\u00e9n\u00e9tiques<\/h4>\n\n\n\n<p>L&rsquo;id\u00e9e est d&rsquo;estimer le risque associ\u00e9 \u00e0 la somme des diff\u00e9rents variants port\u00e9s par un individu. \u00ab La plupart des variants courants ont peu ou pas d&rsquo;effet mais, lorsqu&rsquo;un variant est associ\u00e9 \u00e0 plusieurs autres, ils peuvent ensemble avoir un impact, \u00bb explique <a href=\"https:\/\/www.uni.lu\/lcsb-en\/people\/patrick-may\/\" target=\"_blank\" rel=\"noreferrer noopener\">Dr Patrick May<\/a>, chercheur au <a href=\"https:\/\/www.uni.lu\/lcsb-en\/research-groups\/bioinformatics-core\/\" target=\"_blank\" rel=\"noreferrer noopener\">Bioinformatics Core<\/a> du LCSB. Il a dirig\u00e9 l&rsquo;analyse g\u00e9n\u00e9tique pour cette \u00e9tude avec <a href=\"https:\/\/www.uni.lu\/lcsb-en\/people\/zied-landoulsi\/\" target=\"_blank\" rel=\"noreferrer noopener\">Dr Zied Landoulsi<\/a> qui poursuit : \u00ab Nous utilisons des m\u00e9thodes statistiques et des \u00e9tudes g\u00e9n\u00e9tiques \u00e0 grande \u00e9chelle pour comprendre si chaque variant est li\u00e9 \u00e0 la maladie ou \u00e0 un trait sp\u00e9cifique. En additionnant les effets estim\u00e9s de ces petites modifications sur plusieurs g\u00e8nes, nous pouvons calculer un score de risque et identifier les personnes ayant une probabilit\u00e9 plus \u00e9lev\u00e9e de d\u00e9velopper la maladie de Parkinson. \u00bb<\/p>\n\n\n\n<p>De plus, pour les maladies tr\u00e8s h\u00e9t\u00e9rog\u00e8nes, avec un large \u00e9ventail de g\u00e8nes et de m\u00e9canismes mol\u00e9culaires impliqu\u00e9s, ce type d&rsquo;approche pourrait \u00eatre utile pour stratifier les patients, c&rsquo;est-\u00e0-dire les diviser en diff\u00e9rents groupes. Cela permettrait de mettre en place ce qu\u2019on appelle la m\u00e9decine de pr\u00e9cision. Les patients pr\u00e9sentant un score de risque polyg\u00e9nique \u00e9lev\u00e9 pour les g\u00e8nes qui r\u00e9gulent une fonction biologique donn\u00e9e pourraient par exemple mieux r\u00e9pondre aux traitements ciblant cette fonction-l\u00e0.<\/p>\n\n\n\n<h4 class=\"has-text-align-left wp-block-unilux-blocks-heading\"        id=\"identifier-les-patients-dont-les-mitochondries-sont-affectees\"\n    >\nIdentifier les patients dont les mitochondries sont affect\u00e9es<\/h4>\n\n\n\n<p>\u00ab Nous savons que les mitochondries, qui produisent l\u2019\u00e9nergie dont nos cellules ont besoin, jouent un r\u00f4le dans la maladie de Parkinson. Des \u00e9tudes ont montr\u00e9 que leur dysfonctionnement peut intervenir tr\u00e8s t\u00f4t et \u00eatre impliqu\u00e9 dans le d\u00e9clenchement de la maladie, \u00bb explique <a href=\"https:\/\/www.uni.lu\/lcsb-en\/people\/anne-grunewald\/\" target=\"_blank\" rel=\"noreferrer noopener\">Prof. Anne Gr\u00fcnewald<\/a>, responsable du groupe <a href=\"https:\/\/www.uni.lu\/lcsb-en\/research-groups\/molecular-functional-neurobiology\/\" target=\"_blank\" rel=\"noreferrer noopener\">Molecular &amp; Functional Neurobiology<\/a>. \u00ab Nous avons donc travaill\u00e9 sur l&rsquo;hypoth\u00e8se selon laquelle certains patients pr\u00e9senteraient une combinaison de variants au niveau de diff\u00e9rents g\u00e8nes mitochondriaux se trouvant dans le noyau des cellules, ce qui contribuerait chez eux au processus de neurod\u00e9g\u00e9n\u00e9rescence. \u00bb<\/p>\n\n\n\n<p>En analysant des donn\u00e9es provenant de deux grandes cohortes tr\u00e8s bien suivies, l&rsquo;<a href=\"https:\/\/www.parkinson.lu\/fr\/recherche-participation\/etude-luxembourgeoise-parkinson\" target=\"_blank\" rel=\"noreferrer noopener\">\u00c9tude luxembourgeoise sur la maladie de Parkinson<\/a> et <a href=\"https:\/\/www.zonmw.nl\/sites\/zonmw\/files\/typo3-migrated-files\/COURAGE-PD_Fact_Sheet.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">COURAGE-PD<\/a>, les chercheurs ont calcul\u00e9 des scores de risque polyg\u00e9niques sp\u00e9cifiques aux mitochondries pour plus de 14.000 patients et participants contr\u00f4les. Ils ont ainsi montr\u00e9 que la pr\u00e9sence de multiples mutations au niveau des g\u00e8nes r\u00e9gulant une fonction des mitochondries, \u00e0 savoir la phosphorylation oxydative (OXPHOS), est significativement associ\u00e9e \u00e0 la maladie. \u00ab Ces premiers r\u00e9sultats renforcent l&rsquo;id\u00e9e qu&rsquo;une alt\u00e9ration de la respiration mitochondriale est un facteur important pour la maladie de Parkinson, \u00bb explique <a href=\"https:\/\/www.uni.lu\/lcsb-en\/people\/giuseppe-arena\/\" target=\"_blank\" rel=\"noreferrer noopener\">Dr Giuseppe Arena<\/a>, chercheur au sein du groupe <a href=\"https:\/\/www.uni.lu\/lcsb-en\/research-groups\/translational-neuroscience\/\" target=\"_blank\" rel=\"noreferrer noopener\">Translational Neuroscience<\/a> et l&rsquo;un des premiers auteurs de cette \u00e9tude. \u00ab Ils indiquent aussi que les scores de risque polyg\u00e9niques constituent un outil utile pour stratifier g\u00e9n\u00e9tiquement les patients, nous permettant d&rsquo;identifier efficacement ceux qui pr\u00e9sentent des alt\u00e9rations au niveau des mitochondries. \u00bb<\/p>\n\n\n\n<h4 class=\"has-text-align-left wp-block-unilux-blocks-heading\"        id=\"valider-ces-scores-genetiques-au-laboratoire\"\n    >\n<strong>Valider ces scores g\u00e9n\u00e9tiques au laboratoire<\/strong><\/h4>\n\n\n\n<p>Les chercheurs ont ensuite voulu d\u00e9terminer si les scores de risque calcul\u00e9s correspondaient \u00e0 des caract\u00e9ristiques bien pr\u00e9cises dans les cellules des patients. Ils ont donc r\u00e9alis\u00e9 plusieurs tests sur des cellules cutan\u00e9es donn\u00e9es par des participants de l&rsquo;\u00c9tude luxembourgeoise sur la maladie de Parkinson et sur des cellules nerveuses obtenues en laboratoire \u00e0 partir de ces \u00e9chantillons de peau. Cet essai, dirig\u00e9 par <a href=\"https:\/\/www.sheffield.ac.uk\/smph\/people\/neuroscience\/oliver-bandmann#:~:text=My%20research%20focuses%20on%20movement,would%20slow%20down%20disease%20progression.\" target=\"_blank\" rel=\"noreferrer noopener\">Prof. Bandmann<\/a> de l&rsquo;Universit\u00e9 de Sheffield et <a href=\"https:\/\/www.ucl.ac.uk\/ion\/research\/our-departments\/clinical-and-movement-neurosciences\/people\/prof-t-foltynie\" target=\"_blank\" rel=\"noreferrer noopener\">Prof. Foltynie<\/a> de l&rsquo;University College de Londres, se concentrait sur un compos\u00e9 connu pour restaurer le bon fonctionnement des mitochondries. Les r\u00e9sultats ont montr\u00e9 que les patients pr\u00e9sentant un score de risque \u00e9lev\u00e9 r\u00e9pondaient plus efficacement \u00e0 ce traitement, corroborant le potentiel de cette approche de stratification g\u00e9n\u00e9tique.<\/p>\n\n\n\n<ul class=\"wp-block-unilux-blocks-custom-buttons btn-list\"><\/ul>\n<\/div>\n<\/div>\n<\/div>\n<\/section>\n\n\n<p>\u00ab C\u2019est vraiment encourageant de voir que l&rsquo;utilisation de scores de risque sp\u00e9cifiques aux mitochondries permettra de s\u00e9lectionner des groupes de patients plus homog\u00e8nes pour des essais cliniques centr\u00e9s sur les mitochondries, \u00bb souligne Prof. Anne Gr\u00fcnewald. \u00ab Il s&rsquo;agit d&rsquo;un nouvel outil pr\u00e9cis pour identifier les patients qui b\u00e9n\u00e9ficieront le plus de th\u00e9rapies cibl\u00e9es. Par ailleurs, comme le dysfonctionnement des mitochondries est souvent associ\u00e9 \u00e0 une apparition pr\u00e9coce de la maladie, les personnes pr\u00e9sentant des scores tr\u00e8s \u00e9lev\u00e9s pourraient \u00e0 l&rsquo;avenir b\u00e9n\u00e9ficier de traitements pr\u00e9ventifs. \u00bb<\/p>\n\n\n\n<p>\u00ab Ces r\u00e9sultats constituent une \u00e9tape importante pour la m\u00e9decine de pr\u00e9cision pour la maladie de Parkinson et sont particuli\u00e8rement pertinents pour les chercheurs engag\u00e9s dans le transfert de la recherche fondamentale vers des applications th\u00e9rapeutiques concr\u00e8tes, \u00bb conclut <a href=\"https:\/\/www.uni.lu\/lcsb-en\/people\/rejko-kruger\/\" target=\"_blank\" rel=\"noreferrer noopener\">Prof. Rejko Kr\u00fcger<\/a>, responsable du groupe Translational Neuroscience et coordinateur du <a href=\"https:\/\/parkinson.lu\/fr\/\" target=\"_blank\" rel=\"noreferrer noopener\">Centre national d&rsquo;excellence pour la recherche sur la maladie de Parkinson<\/a>. \u00ab Ce travail a \u00e9t\u00e9 rendu possible par l&rsquo;\u00c9tude luxembourgeoise sur la maladie de Parkinson dont la conception a permis un d\u00e9pistage g\u00e9n\u00e9tique mais aussi des analyses fonctionnelles sur des cellules provenant des m\u00eames patients. La strat\u00e9gie de stratification bas\u00e9 sur ces scores de risque va ouvrir la voie \u00e0 de nouvelles collaborations avec de partenaires qui souhaitent tester des compos\u00e9s cibl\u00e9s pour
