{"id":824,"date":"2024-04-22T09:54:57","date_gmt":"2024-04-22T07:54:57","guid":{"rendered":"https:\/\/www.uni.lu\/lcsb-fr\/?post_type=news&#038;p=824"},"modified":"2025-02-17T15:37:32","modified_gmt":"2025-02-17T14:37:32","slug":"predire-arythmie-30-minutes-avant-son-declenchement","status":"publish","type":"news","link":"https:\/\/www.uni.lu\/lcsb-fr\/news\/predire-arythmie-30-minutes-avant-son-declenchement\/","title":{"rendered":"Pr\u00e9dire l&rsquo;arythmie 30 minutes avant qu&rsquo;elle ne se d\u00e9clenche"},"content":{"rendered":"\n<section class=\"wp-block-unilux-blocks-free-section section\"><div class=\"container xl:max-w-screen-xl\">\n<p>La fibrillation atriale est la forme d\u2019arythmie cardiaque la plus r\u00e9pandue dans le monde, avec environ <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0735109720377755?via%3Dihub\" target=\"_blank\" rel=\"noreferrer noopener\">59 millions de personnes concern\u00e9es en 2019<\/a>. Ce rythme cardiaque irr\u00e9gulier est associ\u00e9 \u00e0 des risques accrus d&rsquo;insuffisance cardiaque, de d\u00e9mence et d&rsquo;accident vasculaire c\u00e9r\u00e9bral. Elle repr\u00e9sente une lourde charge pour les syst\u00e8mes de sant\u00e9, ce qui fait de sa d\u00e9tection et prise en charge pr\u00e9coce un objectif majeur. Des chercheurs du <a href=\"https:\/\/www.uni.lu\/lcsb-fr\/\" target=\"_blank\" rel=\"noreferrer noopener\">Luxembourg Centre for Systems Biomedicine<\/a> (LCSB) de l&rsquo;Universit\u00e9 du Luxembourg ont r\u00e9cemment mis au point un mod\u00e8le capable de pr\u00e9dire le passage d&rsquo;un rythme cardiaque normal \u00e0 la fibrillation atriale. Il peut donner l\u2019alerte en moyenne 30 minutes avant l&rsquo;apparition de la fibrillation, avec une pr\u00e9cision d&rsquo;environ 80 %. Ces r\u00e9sultats, <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S2666389924000783?via%3Dihub\" target=\"_blank\" rel=\"noreferrer noopener\">publi\u00e9s dans la revue scientifique <em>Patterns<\/em><\/a>, ouvrent la voie \u00e0 l\u2019int\u00e9gration de ce type de mod\u00e8le dans les technologies portables, permettant une prise en charge pr\u00e9coce et de meilleurs r\u00e9sultats pour les patients.<\/p>\n\n\n\n<p>En cas de fibrillation atriale, les cavit\u00e9s sup\u00e9rieures du c\u0153ur battent de mani\u00e8re irr\u00e9guli\u00e8re et ne sont pas synchronis\u00e9es avec les ventricules. Le retour \u00e0 un rythme r\u00e9gulier peut n\u00e9cessiter des proc\u00e9dures plus ou moins lourdes, qu&rsquo;il s&rsquo;agisse d\u2019utiliser un choc \u00e9lectrique pour que le c\u0153ur retrouve un rythme sinusal normal ou de proc\u00e9der \u00e0 l\u2019ablation d\u2019une zone sp\u00e9cifique responsable des signaux anormaux. Pouvoir pr\u00e9dire un \u00e9pisode de fibrillation atriale suffisamment t\u00f4t permettrait aux patients de prendre des mesures pr\u00e9ventives pour maintenir un rythme cardiaque stable. Cependant, avec les m\u00e9thodes actuelles bas\u00e9es sur l&rsquo;analyse du rythme cardiaque et des donn\u00e9es issues d\u2019un \u00e9lectrocardiogramme (ECG), la fibrillation atriale ne peut \u00eatre d\u00e9tect\u00e9e que juste avant son apparition, ne laissant pas assez de temps pour alerter les patients.<\/p>\n\n\n\n<p>\u00ab Notre travail s&rsquo;\u00e9carte de cette approche pour adopter un mod\u00e8le de pr\u00e9diction plus prospectif, \u00bb explique le <a href=\"https:\/\/www.uni.lu\/lcsb-en\/people\/jorge-goncalves\/\" target=\"_blank\" rel=\"noreferrer noopener\">professeur Jorge Goncalves<\/a>, responsable du groupe <a href=\"https:\/\/www.uni.lu\/lcsb-en\/research-groups\/ai-modelling-prediction\/\">AI Modelling and Prediction<\/a> au LCSB. \u00ab Nous avons utilis\u00e9 des donn\u00e9es de fr\u00e9quence cardiaque pour entra\u00eener un mod\u00e8le d&rsquo;apprentissage profond capable de reconna\u00eetre diff\u00e9rentes phases \u2013 rythme sinusal, pr\u00e9-fibrillation et fibrillation atriale \u2013 et de calculer la probabilit\u00e9 que le patient connaisse un \u00e9pisode imminent. \u00bb \u00c0 l&rsquo;approche de la fibrillation atriale, cette \u00ab probabilit\u00e9 de danger \u00bb augmente jusqu&rsquo;\u00e0 ce qu&rsquo;elle franchisse un seuil sp\u00e9cifique et que le mod\u00e8le donne l\u2019alerte.<\/p>\n\n\n\n<p>Ce mod\u00e8le d&rsquo;intelligence artificielle, appel\u00e9 WARN (Warning of Atrial fibRillatioN), a \u00e9t\u00e9 entra\u00een\u00e9 puis test\u00e9 sur des enregistrements de 24 heures recueillis aupr\u00e8s de 350 patients \u00e0 l&rsquo;h\u00f4pital Tongji (Wuhan, Chine). Il a donn\u00e9 des alertes pr\u00e9coces, en moyenne 30 minutes avant le d\u00e9but de la fibrillation atriale, avec une grande pr\u00e9cision. Le fait que son bon fonctionnement ne n\u00e9cessite pas une forte puissance de calcul en fait un outil id\u00e9al pour l&rsquo;int\u00e9gration dans les technologies portables. L&rsquo;objectif \u00e0 long terme est que chaque patient puisse suivre son rythme cardiaque en continu et b\u00e9n\u00e9ficier d\u2019alertes pr\u00e9coces lui donnant suffisamment de temps pour agir : prendre des m\u00e9dicaments antiarythmiques ou utiliser des traitements cibl\u00e9s afin de pr\u00e9venir le d\u00e9clenchement de la fibrillation atriale. Cela permettrait de r\u00e9duire les proc\u00e9dures d&rsquo;urgence et d&rsquo;am\u00e9liorer la qualit\u00e9 de vie des patients.<\/p>\n\n\n\n<p>\u00ab \u00c0 l&rsquo;avenir, nous allons nous concentrer sur le d\u00e9veloppement de mod\u00e8les personnalis\u00e9s. L&rsquo;utilisation quotidienne d&rsquo;une montre connect\u00e9e fournit constamment de nouvelles informations sur la dynamique cardiaque d\u2019une personne donn\u00e9e. Financ\u00e9es par le <a href=\"https:\/\/www.fnr.lu\/\" target=\"_blank\" rel=\"noreferrer noopener\">Luxembourg National Research Fund (FNR)<\/a>, les DTU offrent une formation de haute qualit\u00e9 aux \u00e9tudiants en doctorat, se concentrant notamment sur l\u2019aspect interdisciplinaire de la recherche scientifique.<\/p>\n\n<p>Dans le cas de CriTiCS, onze doctorants ont \u00e9tudi\u00e9 la mani\u00e8re dont un \u00e9v\u00e8nement catastrophique se d\u00e9clenche, qu\u2019il s\u2019agisse d\u2019un krach boursier ou de l&rsquo;apparition d\u2019une maladie.
